Participation Waiver

 

I hereby certify that I am the adult parent or guardian of the participant, a minor child under the age of eighteen years, and I consent to his/her participation in recreational skate activities organized by Cariuma. I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury, property loss or damage) of (1) said minor child’s participation in recreational activities and (2) his/her use of skateboard. I recognize my responsibility to ensure that said minor child participates only in those activities for which he/she has the required skills, qualifications, training and physical conditioning. I understand that Cariuma shall have no responsibility to pay for medical treatment and related costs if said minor child is injured. Knowing the risks described above, I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s participation in Cariuma's Skateboarding Activities. To the fullest extent allowed by law, I hold harmless and agree to indemnify Cariuma and its personnel, from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor child’s participation in recreational activities organized by Cariuma resulting from any cause whatsoever, and regardless of fault. I am at least eighteen years of age and have carefully read and freely signed this Liability Waiver and Release Form (Minor Child). I understand and agree that no oral or written representations can or will alter the contents of this document.

Content Creation Waiver

 

I hereby grant permission to the rights of the participant's image, likeness and sound of his/her voice as recorded on audio or video tape without payment or any other consideration. I understand that his/her image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein his/her likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of his/her image or recording. Photographic, audio or video recordings may be used for the online, social and offline purposes.

 

By checking this box, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet. I will be consulted about the use of the photographs or video recording for any purpose other than those listed above. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed. This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only. By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material.
